Fasciolasis là nhiễm trùng gây ra bởi 2 loại sán lá gan F.hepatica hoặc F.gigantica [1]. F.hepatica hay gặp hơn và phân bố rộng khắp, trong khi đó F.gigantica phân bố chủ yếu ở vùng nhiệt đới. Cả 2 loại sán này đều là sán lá, hermaphroditic, có chu kỳ và biểu hiện lâm sàng tương tự nhau. Người là vật chủ ngẫu nhiên.

FASCIOLA HEPATICA — Sán F.hepatica trưởng thành kích thước khá lớn, dẹt, màu nâu và hình lá, 2.5-3cm x 1-1.5cm. Phần đầu to hơn và có xương sống. Sán sống trong đường mật chính và đường mật trong gan của người và động vật. Trứng hình bầu dục, màu vàng nâu và kích thước 130-150 x 60-90 micromet.

Dịch tễ học — Sán lá gan hepatica là một mầm bệnh hay gặp ở cừu và gia súc, chủ yếu ở các vùng nuôi cừu, đặc biệt là một số vùng ở Trung và Nam Mỹ, châu Âu, Trung Quốc, châu Phi và Trung Đông. Một số ca tản phát đã gặp ở Mỹ [1]. Tổng số người nhiễm ước tính là 2,4 triệu ở 60 nước và số người có nguy cơ mắc là trên 180 triệu trên toàn thế giới [2]. Ở một số vùng của Bolivia, tỷ lệ nhiễm ở người lên tới 65-92% [3].

Người nhiễm sán do ăn rau (rau cải xoong) trồng ở các vùng nuôi cừu. Các rau trồng nước ngọt khác cũng có thể truyền bệnh, ví dụ rau diếp, bạc hà…Đôi khi người cũng nhiễm bệnh do uống nước nhiễm ấu trùng (metacercariae) [4]. Một số vụ dịch cũng đã được mô tả [5].

Vòng đời — Mặc dù cừu và gia súc là vật chủ quan trọng nhất của F.hepatica, các súc vật khác như ngựa, lạc đà, hươu, thỏ cũng có thể nhiễm. Trong cơ thể vật chủ, sán trưởng thành nằm trong đường mật chính và đẻ trứng. Trứng qua cơ thắt Oddi, vào ruột non và ra ngoài qua phân. Sự phát triển sau đó xảy ra trong môi trường nước, và các miracidia nở ra từ trứng sau khoảng 9-15 ngày. Miracidia sau đó sẽ ký sinh vào một số loại ốc (Lymnaea) và đây là vật chủ trung gian thứ nhất.

Khi ký sinh ở ốc, các miracidia sinh sản và chuyển thành cercariae trong 4-7 tuần. Cercariae sau đó sẽ nhanh chóng thoát ra khỏi ốc, đóng kén và dính lên nhiều loại rau thủy sinh, phát triển thành metacercariae. Khi người hoặc các vật chủ khác ăn phải rau, các metacercariae sẽ thoát khỏi kén trong tá tràng và giải phóng ra ấu trùng. Ấu trùng xuyên qua thành ruột vào ổ bụng, sau đó xuyên qua bao gan và xâm nhập vào gan, cuối cùng nằm trong đường mật. Sán phát triển thành dạng sán trưởng thành trong đường mật; khoảng 12 tuần sau khi nhiễm, các sán đó mới bắt đầu đẻ trứng. Do đó chẩn đoán trong vòng 3-4 tháng đầu là tương đối khó khăn.

Tuổi thọ của sán F.hepatica trưởng thành trong cơ thể người hiện vẫn chưa rõ nhưng ước tính là từ 9-13 năm. Số lượng sán tới đường mật và phát triển thành dạng trưởng thành về sinh sản thường là ít. Mức độ nhiễm sán tăng trong những năm mưa nhiều do số lượng ốc tăng lên và các kén cercariae sống lâu hơn [6].

Bệnh sinh — Các metacercariae khi di chuyển sẽ phá hủy nhu mô gan và đường di cư của sán là một đường hoại tử và xơ hóa. Mức độ phá hủy gan tùy thuộc lượng sán. Sán trưởng thành có thể gây tắc bán phần đường mật, dày đường mật, dãn và xơ hóa các nhánh đường mật lớn (proximal biliary tree) [7]. Sán lạc chỗ cũng gây thâm nhiễm bạch cầu ái toan và đơn nhân kèm theo phá hủy tổ chức.

Biểu hiện lâm sàng — Nhiều người nhiễm sán thường biểu hiện nhẹ [8], tuy nhiên khi nhiễm sán số lượng lớn thì biểu hiện sẽ nặng. Các triệu chứng điển hình của nhiễm sán có thể chia thành nhiều giai đoạn: giai đoạn cấp hoặc giai đoạn gan, giai đoạn mạn tính hoặc giai đoạn mật, và sán lạc chỗ hoặc sán ở họng.

Giai đoạn cấp —Giai đoạn sớm khi ký sinh trùng di cư xuyên qua gan thường kèm theo biểu hiện sốt, đau HSP và gan to. Triệu chứng thường xuất hiện trong vòng 6-12 tuần sau khi nuốt phải ấu trùng. Các triệu chứng khác có thể xảy ra là chán ăn, buồn nôn, nôn, đau cơ, ho và nổi mày đay. Đôi khi vàng da. Trong một số trường hợp giai đoạn này có biến chứng chảy máu đường mật hoặc u máu dưới bao gan [4]. Các triệu chứng cấp tính thường biến mất sau 1 vài tuần hoặc 1 vài tháng (thường 6 tuần). Tuy nhiên trong các ca nhiễm nhiều sán, hoại tử gan diện rộng có thể xảy ra.

Tăng bạch cầu ái toan rất thường gặp trong giai đoạn sớm này. Triệu chứng ở các cơ quan khác đôi khi cũng xảy ra, có lẽ là do cơ chế dị ứng hoặc miễn dịch [6]. Đôi khi có tràn dịch màng phổi phải tăng bạch cầu ái toan hoặc hội chứng kiểu Loeffler [3]. Viêm ngoại tâm mạc, rối loạn dẫn truyền, hội chứng màng não, dấu hiệu thần kinh khu trú hoặc co giật cũng đã được mô tả nhưng hiếm.

Một bài tổng quan về fascioliasis ở bệnh nhân các nước phát triển cho thấy triệu chứng của giai đoạn cấp thường xuất hiện trước khi chẩn đoán bệnh khoảng 1 tháng trong số 73% số bệnh nhân [6]. The diagnosis of fascioliasis should be entertained in patients with lingering symptoms and signs, particularly abdominal pain and hepatomegaly, accompanied by peripheral eosinophilia. Chẩn đoán sán lá gan cần được nghĩ đến ở bệnh nhân có các dấu hiệu và triệu chứng gợi ý, đặc biệt là đau bụng và gan to, kèm theo tăng bạch cầu ái toan ngoại vi. Cần hỏi kỹ tiền sử ăn uống ví dụ ăn rau sống, rau cải xoong…[9].

Giai đoạn mạn tính — Giai đoạn này thường không triệu chứng, nhưng sán trưởng thành có thể gây tắc đường mật chính. Nhiễm sán giai đoạn này có thể gây ra đau bụng do co thắt đường mật, viêm đường mật, sỏi mật và vàng da tắc mật. Nhiễm sán số lượng lớn và kéo dài có thể gây viêm xơ đường mật và xơ gan mật. Đau vùng thượng vị và HSP, ỉa chảy, buồn nôn, nôn, gan to và vàng da là các triệu chứng có thể xuất hiện ở bệnh nhân. Tăng bạch cầu ái toan là một biểu hiện không thường xuyên trong giai đoạn này.

Sán lạc chỗ — Mặc dù sán có ái tính với gan, sán cũng có thể ký sinh ở các cơ quan khác và gây triệu chứng. Liệu sán di cư tới các vị trí bất thường đó theo đường máu hay theo đường mô mềm thì hiện còn chưa rõ. Vị trí hay gặp nhất là tổ chức dưới da ở thành bụng, tuy vậy phổi, tim, não, cơ, đường sinh dục và da đều có thể là vị trí ký sinh [10]. Đau do sán di cư là triệu chứng có thể xảy ra, kèm theo các nốt đỏ, ngứa và đau, di chuyển, kích thước 1-6 cm. Apxe nhỏ tại chỗ, hạch to toàn thân đều có thể là triệu chứng [7].

Sán ở họng — Mặc dù hiếm nhưng ở một số vùng như Trung Đông khi người ta ăn gan động vật sống, sán có thể gắn trực tiếp vào đường hô hấp trên hoặc đường tiêu hóa cao, gây viêm họng với biểu hiện phù nề và sung huyết. Đôi khi gây khó thở, gọi là “halzoun”.

Chẩn đoán — Chẩn đoán dựa vào tìm trứng trong phân, dịch hút tá tràng hoặc mật.

Soi hiển vi — Trứng sẽ dễ phát hiện hơn nếu bệnh phẩm được phong phú (concentrated). Cần xét nghiệm nhiều mẫu vì lượng trứng sinh ra thường thấp và thải ra phân từng đợt. Không thấy trứng trong phân không loại được chẩn đoán. Thường rất khó phân biệt giữa trứng của F.hepatica, F.gigantica và sán lá ruột F.buski. Cũng có thể chẩn đoán khi phẫu thuật hoặc nội soi điều trị tắc mật tìm thấy sán trưởng thành.

Trong giai đoạn cấp, không có trứng trong phân, do đó chẩn đoán cần được hướng tới khi có biểu hiện lâm sàng gợi ý và tăng bạch cầu ái toan trong máu ngoại vi. Thiếu máu, rối loạn chức năng gan, tăng máu lắng và tăng gammaglobulin là các biểu hiện có thể gặp khi xét nghiệm. CT trong giai đoạn này có thể gặp hình ảnh giảm tỷ trọng nhỏ, dạng đường hầm chia nhánh, đường kính 1-10mm, chính là đường đi của sán chưa trưởng thành (ấu trùng) trong gan.[3]. Soi ổ bụng có thể thấy các nốt khá đặc hiệu ở bao gan. Xét nghiệm tìm kháng nguyên trong máu hoặc phân có thể phát hiện sán vài tuần trước khi có trứng trong phân [11,12].

Đối với sán lạc chỗ, xét nghiệm phân thường âm tính. Xét nghiệm dựa vào huyết thanh chẩn đoán hoặc tìm thấy sán trong bệnh phẩm phẫu thuật [13].

Huyết thanh chẩn đoán — Các xét nghiệm ngưng kết gián tiếp, cố định bổ thể, điện di miễn dịch, miễn dịch huỳnh quang, và ELISA đều đã lưu hành trên thị trường. Mọi xét nghiệm đều có độ nhạy tốt, nhưng nhiều xét nghiệm có độ đặc hiệu thấp và phản ứng chéo với các ký sinh trùng khác. Xét nghiệm huyết thanh thường dương tính trong giai đoạn sớm khi sán qua gan, do đó ứng dụng tốt để chẩn đoán khi trứng chưa có trong phân và chẩn đoán sán lạc chỗ. Xét nghiệm ELISA sử dụng kháng nguyên đặc hiệu loài hiện đã thay thế tất cả các kỹ thuật còn lại do nhạy, cho kết quả nhanh và định lượng được [14,15]. Tuy vậy, mặc dù điều trị làm giảm hiệu giá ELISA, kháng thể vẫn còn tồn tại nhiều năm sau nhiễm sán [16].

Một nghiên cứu cho thấy ELISA dùng kháng nguyên chiết từ sán trưởng thành có độ nhạy 100% và độ đặc hiệu 97.8% [17]. Các nghiên cứu khác cho kết quả xét nghiệm ELISA sàng lọc (Falcon ELISA) và xét nghiệm miễn dịch thấm gắn men có độ nhạy tương ứng là 95% và 100%, còn độ đặc hiệu thì chưa xác định được [14,18].

Một kỹ thuật ELISA tìm kháng nguyên lưu hành đã được sản xuất và có Se 100%, Sp 98% [19]. Bệnh nhân nhiễm nhiều sán có nồng độ kháng nguyên cao hơn [20]. Xét nghiệm dương tính trước khi trứng ra phân, và do đó rất tốt để chẩn đoán nhiễm sán cấp. Xét nghiệm âm tính ở 65% bệnh nhân 1 tháng sau điều trị [21,22].

CDHA — Tốt nhất là CT với hình ảnh điển hình mô tả ở trên. Đôi khi còn thấy dày bao gan, u máu dưới bao gan và vôi hóa nhu mô gan [23].

Siêu âm và ERCP có hiệu quả tốt đối với giai đoạn mạn tính, có thể phát hiện sán cử động trong đường mật và túi mật, thường kèm theo sỏi [24]. Đôi khi thấy dày đường mật chính [25]. MRI cũng có thể dùng nhưng hiệu quả không hơn gì CT hoặc siêu âm [23].

Sinh thiết gan rất hiếm khi phát hiện trứng hoặc sán trưởng thành [26]. Các thay đổi mô bệnh học gợi ý tới fascioliasis là tinh thể Charcot-Leyden, bạch cầu ái toan và các ổ vôi hóa [27].

Điều trị — Triclabendazole hiện đã thay thể bithionol trong điều trị sán lá gan lớn [28].

Triclabendazole — Liều triclabendazole là 10 mg/kg trong 1-2 ngày. Thuốc có tác dụng với cả sán trưởng thành và chưa trưởng thành. Hấp thu tốt hơn sau ăn.

Bithionol — Bithionol (30 to 50 mg/kg in three divided doses on alternate days for 10 to 15 doses) [29]. Tác dụng phụ bao gồm buồn nôn, nôn, đau bụng và ngứa. Tỷ lệ khỏi bệnh từ 50-90% [6,30]. Đôi khi cần lặp lại 1 đợt điều trị [18].

Trong một nghiên cứu tiến hành ở Ai Cập, triclabendazole đã được dùng để điều trị 40 bệnh nhi nhiễm fasciola, 8 trong số đó đã thất bại với các điều trị khác [31]. Sau một liều duy nhất 10 mg/kg, 78% đã khỏi hoàn toàn, số còn lại cần một liều thứ 2 mới khỏi. Không thấy có tác dụng phụ nào [18,32-34].

Các điều trị khác —Giống như các loại sán lá khác, F.hepatica không đáp ứng với praziquantel. Mebendazole và albendazole cũng không có tác dụng. Emetine hydrochloride đã từng được dùng trong quá khứ nhưng do có nhiều tác dụng phụ nên giờ cũng không còn sử dụng nữa. Nitazoxanide (500 mg bid trong 7 ngày) cũng có tác dụng với tỷ lệ khỏi là 82.4% trong một nghiên cứu [35]. Một nghiên cứu ở người lớn và trẻ em ở vùng phía bắc của Peru đã đánh giá hiệu quả và tính an toàn của liệu trình 7 ngày nitazoxanide (100 mg bid nếu 2-3 tuổi, 200 mg bid nếu 4-11 tuổi và 500 mg bid nếu >12 tuổi), so sánh với giả dược [36]. Nitazoxanide dung nạp tốt và chữa khỏi 40% trẻ em và 60% người lớn, so với 6% nhóm giả dược (p<0.05).

Trong giai đoạn cấp, 1 liệu trình corticoid ngắn ngày có thể áp dụng cho bệnh nhân có triệu chứng nặng, kèm theo với điều trị diệt sán [37].

Biến chứng và theo dõi — Các biến chứng như viêm đường mật cần được điều trị bằng KS và phẫu thuật. Tắc mật cần được giải quyết bằng ERCP và có thể lấy được sán ra khỏi đường mật [24,25,38].

Khi chẩn đoán sán lá gan ở một thành viên trong gia đình thì các thành viên khác có nguy cơ nhiễm cũng cần được khám. Các ca không triệu chứng cần được điều trị để hạn chế biến chứng sau này.

Theo dõi sau điều trị bao gồm theo dõi sự biến mất của hiện tượng tăng bạch cầu ái toan, trứng trong phân và giảm hiệu giá huyết thanh. Theo dõi sự tiến triển của hình ảnh siêu âm đường mật cũng có giá trị [39].

Dự phòng —Tránh ăn rau sống. Việc diệt các loại ốc, thân mềm là không thực tế. Điều trị sán ở các súc vật nhiễm sán cũng có ích kiểm soát bệnh

Các phân tử tổng hợp từ ký sinh trùng đã đem lại sức đề kháng phần nào trong các thực nghiệm [3]. Các nghiên cứu vacxin trên động vật đã thấy có sự giảm số sán nhiễm và giảm đẻ trứng khoảng 70%, nhưng vacxin cho người hiện chưa có [40].

FASCIOLA GIGANTA — F.gigantica tương tự như F.hepatica, tuy sán và trứng có lớn hơn về kích thước (sán có thể tới 7.5cm và trứng 190x90 micromet). Sán gây bệnh ở động vật nuôi ở các vùng nhiều và cận nhiệt đới, đặc biệt châu Phi, Tây TBD, Hawaii và Đông Nam Á. Vật chủ chính và trung gian tương tự F.hepatica.

F.gigantica có vòng đời và đặc điểm gây bệnh giống F.hepatica, bệnh cảnh lâm sàng tương tự. Trứng thường không có trong phân và huyết thanh chẩn đoán thì không khẳng định được. Do đó, chỉ dựa vào vùng địa lý hoặc lấy trực tiếp được sán trưởng thành thì mới khẳng định hoặc loại trừ chẩn đoán. Tuy vậy, 2 nghiên cứu gần đây cho thấy các kháng nguyên đặc hiệu của F.gigantica có thể phát hiện được bằng ELISA không trực tiếp hoặc miễn dịch thấm với Se và Sp đạt tới 100% [41,42]. Một phản ứng PCR kiểu đa hình hạn chế chiều dài (PCR-restriction fragment length polymorphism (RFLP)) đã được dùng để phân biệt F.gigantica và F.hepatica nhưng chưa được sử dụng rộng rãi [43].
